My question is - why would you ever take a unit of three?

Am I correct in assuming that simply losing one would be enough to break the unit? 1BM for taking fire, 1BM for losing a model, 2 models left = SQUAD BROKEN! Right? Am I missing something?

What am I missing here? How do people use Knight formations of three?

Nope, you aren't missing anything. Basically, a formation of 6 doesn't really perform that well and is expensive. Most of the time they are 1 shot weapons. So for a while we had the option for 3 to let folks bring the forge knights without sinking a ton of points. If you are using the latest version of the list (3.2) you'll see that the formation of three knights is no longer an option.

I'd be keen on removing the entire formation since almost everyone uses the paladin model to represent them and the forge knight is a fan unit, but too many people would prefer they stay. As they are, they are difficult choice to use well and once they break, expect to see them vanish.

I see. So they are included in the list for nostalgia's sake?

The point cost seems awfully steep for such a useless unit. Sorry, I don't know much at all about the list making process, but, wouldn't it make sense to lower the cost?

They are currently 375 points for a unit of 6 models. They do have reinforced 4+ armor and an Invulnerable save. That makes them fairly durable unless they get hit with TK weapons so there's pretty good odds you'll get them to a target and they give you 6x regular 4+ attacks and 6x MW4+ attacks which is pretty brutal. They also have infiltrator so they can charge 40cm and pick out units to attack since they ignore ZOC in the formation they are attacking.

So at least stats wise they are probably close to what their point cost should be. The problem is their effectiveness is very situational and they quickly lose effectiveness once they engage. In a list that already has expensive core choices you usually don't have a lot of points free to use on units like the forge knights. That means you don't see them often compared to cheaper options like Thunderbolts and Sentinels. For nearly the same point cost I can gain two additional activations over taking the knights that give me air cover and scout ZOC to protect titans from assault. AMTL is already pretty low on the activation count so you can see the appeal.

Then why not ditch unit size 3 and make it 4 or 5? That might make it more usuable.

If you look at the current version of the list the forge knights only come in a formation of 6 now :)
