I've had a couple of requests for this and yes I will share the template, however it's not currently in a format I'm happy with (I basically figured it out as I went and it's not nearly as 'template like' as I would prefer, it needs a lot of work to edit and change)

I will get to working on it again in the next couple of weeks if people are prepared to wait, alternatively I knocked it up using nothing more than a couple of hours and MS Word and most people could do the same with minimal effort (and probably make it look much nicer than mine!)

it's not in a useable format I'm afraid, I've tried getting it sorted but really it needs starting again, I didn't *really* know what I was doing when I started the project and don't have time to re-do them at present

Largo made some much better ones I suggest using those instead for the time being
