Technically, the "place BM for coming under fire" is the very first step listed in the order of play for firing under 1.9.3. However, checking to see if the formation is broken is a separate step, after the firing is resolved.
1.9.3:

II – Roll to hit.
III – Allocate hits, make saving throws and remove casualties.
IV – Place additional Blast markers for casualties and check to
see if the enemy formation is broken.
Personally, I find that needlessly confusing and everyone I know just places all the BMs at the end.
There was one game I recall where a single unit was in range and my opponent argued that it should be destroyed with a hackdown hit when the BM was placed. That would, of course, limit fire to a single casualty, rather than 3 casualties (1 kill, 1 BM for kill, 1 BM for coming under fire).
