Vermis, Tycho lead common or garden Blood Angels - he was assigned to the Death Co at Armageddon when he went, as you put it, doolally. And was offed by the Orks after the standard Imperial heroic last stand.

Thank you all  Due to the damn flash of my camera the banners (shrink and print ) look a bit bright and the Melta barrel of Tychos combi-weapon looks a bit blobby
