2 x Twin Sponson Multi-meltas obviously get 2 x MW4+. However, do they also receive on their assaults +1 attacks or +2 attacks since it is 2 x the small arms weapon?

Also, if you have a faithful formation and they break, but don't rally, do they remain fearless ala faihful into the next turn, and so on until they rally and activate on the following turn?

As far as I can tell it's a yes on the meltas/+2 attacks, the chapels are extremely good in assault but like warlord titans their speed means they rarely get in range, I've used them quite a bit now and they've only used their meltas once or twice. It's doubly nasty that they can protect each other from CC by choosing to use their FF attacks to attack units in btb with the other chapel.

As for Faithful you're correct that they remain faithful/fearless until they act again so broken units that fail to rally stay fearless. It's a point my opponents continually bring up, I'd like to see it changed so that they lose it on a failed rally which would seem an appropriate punishment for failing in their duty to the Emperor.

I hope you don't mind me answering this, I hope Zombo will take a moment to respond as well.

As CaptainSenioris says, they receive two extra MW attacks in a FF.

Yes, for now they retain the bonuses on a failed rally check, but I'm willing to change that.
