HLS Models has the 12x40s, you might have to e-mail them to do an order though.

I can laser cut plasti-card if you want to go that route, but that means no beveled edges.

15cm - 250m
30cm - 500m
45cm - 1km
60cm - 2km
75cm - 4km (horizon)
90cm - 8km
120cm - 32km
